Placing an Order
- Review the Specimen Requirements and complete a Test Requisition Form (TRF)
- TRF can be filled out online and printed for submission
- TRF must be signed by HCP and dated
- Completed TRF can be submitted via email or fax. Please include:
- The Pathology Report (if available)
- If the Pathology Report is not available at time of order, SkylineDx Customer Service will reach out to obtain the report
- Patient insurance information
- SkylineDx Customer Service will contact the pathology lab or ordering physician to obtain the specimen, and provide specimen kits and shipping materials
- See Specimen Requirements for shipping instructions
- The patient Test Report will be returned via secure email or fax as directed
- Test results delivered within 5 business days upon receipt of a compliant sample
